It’s Not Like It’s An Emergency

As you will hear, sooner or later, the International Association of Firefighters Local 998 (that’s our firefighters) are spending $20,000+ of their own money to commission an outside analysis of Stratford’s Emergency Medical Services. As we know from from the last episode, the Harkins Administration hired a consulting firm (CBG Assoc) to conduct an analysis of SEMS (Stratford EMS). It turns out that one of the partners, Jason Perillo, is the Republican State Representative in Shelton and the Director of Valley Emergency Medical Service, the regional paramedic provider which serves the Lower Naugatuck Valley. Closely associated with Harkins and Adam Bauer (remember him?), CBG recommended the hiring of an EMS Chief elevated in parity to the Police Chief and Fire Chief (our Fire Chief, by the way, is responsible for EMS now).

The Town of Stratford has accepted resumes for that position. We truly hope that Jason Perillo’s (R-Shelton) is not one of them.

In the meantime, Local 998 has decided that a non-partisan and professional consultant is required. That may be so. In response, Mayor Harkins has ordered that none of the department heads in the Town of Stratford may speak with the consultant. Further, that any documents sought by this consultant not be provided unless obtained by a Freedom of Information request. James Miron would be proud of that move. It is so Miron-like.

But let’s get away from the politics just for a moment. Not to worry, only for a moment. The administration of the volunteer Stratford EMS has been so abysmally poor that no one will volunteer any longer. A very disturbing amount of calls on a daily basis must be transferred to AMR, a commercial ambulance service with secondary responsibility for Stratford. This causes delays in response for Stratford residents requiring emergency medical services and is nothing less than life threatening. The situation is so bad that no one keeps track of the average response time in this Town.

Now, back to politics as promised. The poor quantities of volunteers and the extended response times were known before Harkins became mayor and have continued with no foreseen improvement since he has been mayor. In fact, in the months since his own consultant’s study nothing has changed at all.

At the very basic level of Town government, providing emergency services to residents comes first. In the 11 months he has been in office, Mayor Harkins has done literally nothing about the Chiefs appointed by Miron or the quality of those services. Obviously, Local 998 has had enough and so have we.

    And yes, you lost to the candidates selected for us by the Republican and Democratic Town Committees. Once again, we were given the poor choice by these “committees” between the reluctant and the insane. (Harkins v. Miron).

    What we did not know was that the reluctance would turn into the granite-like maintenance of the status quo from Miron’s disastrous administration. We still have an out of control Police Chief who persecutes officers based upon personal vengeance, a Fire Chief who continues to lie about almost everything and denies responsibility for anything and an EMS Administrator who actively punishes volunteers for volunteering.

    Residents’ lives depend upon some sort of real decisions and actions. It is not sufficient to be a placeholder for another three years.

    These issues are clear and defined. Yet it remains that for us residents the best course of action in a medical emergency is to call a cab.

    When I first started going to Mr. P’s about 16 years ago, it was because
    my washer / dryer broke and I awaited doing laundry.
    They had a HOT and PERSONABLE RED HEADED bartender who moved like
    she was wearing roller skates and a rocket up her butt.
    They had great weekend lunch / dinner specials.
    I became absorbed with a college football game, when suddenly the lights
    lowers and music boomed on.
    I turned around and there were many women (and guys) around.
    Many opportunities soon presented themselves…..

    However, there were often so many tattoos and body piercings that I
    thought I was also in the “Star Wars Cantina.”

    Later I found out Mr. P’s was aka “cocaine mountain.”
    It was culture shock.

    However, despite the problems of the people there, I found they have
    more decency, integrity, humanity, and friendliness than found in town
    hall, or among many of the so call powers of Town Committess.

    Sorry Jeze, but I think the Town Committees would agree: They represent the INTERESTS of THEIR CONSTITUENTS on Town Committees!

    If you go to the Video Tape and review my Letters to the Editor I have been
    less kind to the Mirons, but certainly NOT favoring certain Republicans.
    I think you would conceed that.

    What you are unlikely to conceed is many problems predated Mayor Miron.
    You may accept that his involvement as 1995 assistant Town Attorney along
    with contributions of Dick Miron, Burturlas, and many others, have caused
    huge problems for the people of Stratford.

    You are unlikely to accept the Voccolas and others in both parties and those
    outside of stratford have contributed to town problems.

    All politics in america needs an enema.
    All politics in america should reflex improvements that should have been
    implemented the last 50 years.

    I am not going to get into pensions here. There is a whole topic dedicated (for you) just for that.

    Let’s try to focus here. The topic is about EMS and Miron’s administrators left in place by Harkins.

    If you look at Stratford’s org chart http://www.townofstratford.com/Upload/147/16273/24818/15.pdf you will see that EMS is directly under Fire Chief Cavanaugh’s command already. It has been so for as long as anyone can remember. Reasonably, you have to ask yourself how it got so out of control as to threaten the lives of Stratford residents. However, I don’t think there is a reasonable answer. In this case it probably just settles into irresponsibility and incompetence. We have a fire chief who is not even certified as an EMT in charge of EMS.

    What makes the situation even sadder is that we have EMT’s, Paramedics and Firefighters who only want to save people’s lives. (The police have their own problems, never knowing who is the next officer to be arrested by Buturla’s friends).

    I have been asked why Firefighters show up at EMS calls in Stratford. The answer is actually pretty simple. Many times when 911 calls are placed, people sometimes forget (or are unable) to unlock their door. Prying it open with a stethoscope rarely works.

    Which reminds me:

    If you are a senior citizen in Stratford or care for one there is one very important thing you can do: Go to a hardware store (like Home Depot or Lowe’s) and buy a lock box for the dwelling key. They are only about $30 and take less than 5 minutes to install. Call the FD and give them the combination. If 911 is ever called they will have access without having to break in. It could save a life for $30.

    Let me clear that up for you. I don’t care one way or the other what any consultants say. We’ve had consultant after consultant in this Town providing opinions that we paid for. They are rarely used for anything other than whatever they were hired to prove or disprove.

    EMS cannot function effectively now. That puts you and me in real danger. We’ve known this for and months…possibly years. When you are sitting there having a heart attack and you wait 15, 20, 25 minutes or more for someone who knows what they’re doing to show up I call that a real problem. If it’s a stroke, you may already be dead.

    What’s more important is that the situation is getting worse, not better. NO ONE is taking any responsibility for this. Not Donna Best the EMS Administrator, not James Cavanaugh the Fire Chief and most shockingly not John Harkins the Mayor.

    Remember, in the months since the first report absolutely nothing has changed. Everything is exactly as it was except we have fewer volunteers now. If Local 998 wants to step up and spend some money on this, I am just fine with that. But at the end of the day (hopefully not at the end of someone’s life) some action must be taken.

    There is nothing I can disagree about with your sentiments.

    I am simply tired of the bovine scatoly and cover stories of all past
    and present administrations creating all kinds of smoke screens.

    If people in politics and town management workers spent time getting
    things done, instead of various posturing, wouldn’t it be much better?

    We have mutual respect for Tom Murray.

    Tom looked me in the eye and told me he absolutely did not know that
    Smeal did not sell direct, but used Distributors, like NEFA.

    I believe Tom.

    We still paid 90 % more than we have to for 2 non custom fire trucks that
    Statford should have had in 2001 – 2002, and about 2006 – 2007.
    AT $ 230,000 BID PRICE for NON CUSTOM fully equiped
    $ 570,000 BID PRICE for CUSTOM fully equiped

    It is my unequivocal belief instead of $ 900,000 BOND, it should have cost
    $ 460,000 as BID for 2 non custom.

    Jeze, where did that extra money go?
    $ 440,000 is almost 1/10 th of 1 Mil – Your Money

    We have a FIREHOUSE cost over $ 7,000,000
    The FIRE HOUSE was supposed to cost under $ 2,000,000

    The $ 5,400,000 extra is 1.2 Mils of Your Money

    Police and Fire O. T. is Budgeted at $ 3,000,000 or 6/10 of Mil

    The Pension Obligation is over $ 18,000,000 Annually = 4 Mils
    The Pension was supposed to pay for itself.
    Unions request – Management Grants
    If Managment is stealing other ways and bribing Union?

    Health Care exceeds 8 Mils, by itself.
    I conceed we need a Cost – but 8 Mils for over priced Health Care,
    where the HSA presenting to the FAC said “We were referred by Norm
    Aldrich and we have the contract for the Board of Ed.

    Tammany Hall was always JOBS and SUPPORT for VOTES, KICK BACKS and
    SUPPORT. I have no problem.

    Ray Voccola told me that he felt Burturla was too greedy!!!
    Ray was no saint and a good party loyalist.
    However I can not believe Ray would have allowed the massive pilfering
    of Town assets and what the two parties have done to the poor, middle
    class, business, and even rich business owners.

    That town had Chromium in the Water.

    Buried under AVCO SAEP are things worse than anything in Raymark.
    It included chromium.

    AVCO is less than 2 miles upstream from (lovely) SHORT BEACH.
    I won’t go in the water there.
    Occasionally I swim/bathe at Long Beach, where I have less reluctance
    with all of the Ships Motor Oil, and other gunk, etc

    Getting past the legitmated aspects, it looks like kickback and payoff.

    Interestingly, Fire Chief Cavanaugh called Ass’t Chief Murray a liar in print in a letter to the Connecticut Post in October of 2009. To this day Cavanaugh has not apologized or been brought to task for it. Apparently it is ok for Chief’s to lie.

    I wonder who will be pocketing it?

    Police want bonding for police station and it seemed to be pretty
    much the same amount of money moved from Police funds?

    What was a hold the line $ 6 – 7 million budget is $ 9.5 million.

    Why do I get the impression the Budget boondoggles are supplemented
    by Bonding for things that were already bonded or will be bonded again?

    Did you really have to go out of your way to take a slap at this blog? Tsk, tsk tsk. That won’t win you many brownie points with Harkins, Chief Cavanaugh or DC Maffet you know. But it is nice to know you read this blog….occasionally.

    While Hansen’s drive to expand the FD membership is obvious, the management of our emergency medical services remains a serious problem. We cannot imagine that Fire Chief Cavanaugh will be a better administrator than Donna Best.

    What remains is that the recruiting and retention of EMT’s and Paramedics is non-existent. It is not a small matter to become an EMT and an even larger effort to attain paramedic status. We have been fortunate in the past to have had some of the finest. However, the future remains bleak and threatens the health and safety of residents.

    I suspect he might have mixed feelings?
    When Rich Weisel worked for the POST, he was mostly first choice
    of the blog. Sylvia thought Rich was the BEST EVER!
    I lobbied for articles from the STAR, from Musante to Kovach as I
    felt their coverage was superior and Kovach, like Tris DeRoma was
    more balanced than Rich or Fred.

    – For a few years I bought some things related to History
    that I wanted to give to the Library, or Schools, or Historic
    Society. Now I jave no doubt they would be stolen!

    I could give some to Sterling House or Southend Community
    center, but the reasons I bought these were to create sets.

    Even the WW II memorial, was taken over in the printing of the
    program book, by the local vultures.

    I have not heard about this person…yet. I know at least three highly qualified and effective candidates who have submitted their resumes. any one of which would make an excellent EMS Chief. All of which also surpass the stated requirements. None of them have been contacted yet.

    If this “applicant” is really saying that, then he/she is obviously not that bright.

    The issue of mutual aid is an issue EMS has been aware of for many years, if it wasn’t the Local 998, It was members of the SEMS speaking out on deficient staffing and why we use other towns to respond to Stratford residents calls for help.

    Fact: Stratford EMS does provide 24×7 paramedic coverage and we always have at least one ambulance on duty. The response numbers I have looked at indicate Stratford EMS responds approx. 82% of Stratford’s medical emergencies. That ranks Stratford EMS on the lower end compared to any EMS Service in Fairfield County. This is not only unacceptable; but Residents and their families deserve and need better. But, having more paramedics is not the answer to this problem.

    The argument being raised is that “EMS doesn’t have enough Paramedics”. To avoid confusion with terms; Emergency Medical Services in Connecticut utilizes Four Levels of Care which are able of providing different skills depending on the severity of the medical emergency. I order of training: Emergency Response Technicians, Emergency Medical Technicians, Advanced Emergency Medical Technicians and finally Paramedics. In my opinion the solution however is not to add more Paramedics, but rather better manage and utilize the staff Stratford EMS presently has.

    Stratford EMS mainly utilizes Paramedics to staff ambulances, rather than in a capacity where their availability is maximized to the full needs of Stratford residents. For the same reason the fire department doesn’t utilize the only on duty Assistant Fire Chief on a fire truck, is the same reasoning EMS needs to adopt for their paramedics, staffing their Paramedics into the EMS’s already owned “Fly-Car’s” would go a long way to ensuring if a Paramedic is needed, it is available. (EMS owns three ford escape hybrid vehicles for this purpose) A fly car is a vehicle that cannot transport someone to the hospital. (State Law), But when Stratford’s fly cars are staffed with a Paramedic it is dispatched only on most serious of calls, allowing for in most cases the Basic Life Support ambulances to work independent without a Paramedic. As an added bonus crews would be able to better train the new EMT recruits we do get and evaluate the skill levels they are expected to perform from peer EMT’s without confusing our new recruits with the job and unique skillset a Paramedic .

    If there are no Stratford EMS ambulances available at a time call is dispatched and there is a fly car available because it has not been needed, The fly car will respond to “stop the clock” and immediately render Basic Life Support or Advanced Life Support care (if needed) while waiting for the mutual aid ambulance. The fly car paramedic also has the ability to ride a call into the hospital with the mutual aid ambulance ensuring Paramedic level service is maintained and on a budgetary level, recoup some of the lost revenue by billing AMR directly for Stratford’s Paramedic being used on their ambulance (contractual part of our mutual aid agreement with AMR).

    EMS’s main problem is a lack of transport ambulances at the peak times of the day. For instance, there are many evening and overnight shifts that are left with only one ambulance. If a paramedic is committed on an ambulance rather than in a fly car the paramedic is now required to respond to every 911 medical call even if a paramedic isn’t required, because he/she is being used to provide the minimum staffing of an ambulance. This means that the call could have been handled very capably by a Basic Life Support ambulance and the paramedic assigned to the ambulance is now unavailable for the time it takes the crew to complete that particular call.

    Hypothetically: having 100 Paramedics on duty in town, WILL NOT help getting someone to the hospital. Having ambulances staffed with EMT’s on duty will. By removing the Paramedics from the normal staffing of ambulances and utilizing their unique skillset in the fly car’s, EMS can cover almost all of its calls…I say almost all because no service in Fairfield county with similar call volume and demographics has a 100% coverage rate. BUT 98-99% coverage is possible if EMS personnel are properly managed and staffed to the needs that Stratford’s call volume demands.
    Stratford EMS has approx: 75-100 “EMT-Basics” or “EMT’s” which include our volunteer and “per diem” staffing versus 15-20 Paramedics. This number includes our five full time employees, per diem and volunteer staff Paramedic If effort was made to make our ambulances exclusively staffed at this “Basic” level. It would free up our paramedics to respond in fly cars to calls with greater efficiency than if they were on an ambulance, resulting in a reduction of “mutual aid”, almost completely.

    “Almost Completely?”

    Yes.

    I am the shift supervisor on duty each and every Saturday from Noon to Midnight. During this time period for 2010 we have a 98.6% coverage rate for ALL of our calls, as of 11/13/2010. (Numbers that I have personally tallied each and every week) 98.6% vs approx.: 82% is a big difference that translates to roughly 500-700 responses from another agency into Stratford. (And as cited in other areas on this blog, a significant loss in potential revenue for Stratford EMS)

    Why is there such a large disparity between these numbers? The reasoning is on Saturdays, my TEAM staff the EMS vehicles completely different then how you will find them during the week. On Saturday from noon till midnight you will find almost exclusively volunteers with the EMS full time Paramedic assigned to the shift, we staff an average of Three (3) “Basic Life Support” (BLS) Ambulances and Two (2) Fly cars with Paramedics (the other paramedic being me) contrast to staffing Monday thru Friday 6am-6pm when there are only two Paramedic ambulances on duty.
    Additionally if circumstances dictated an immediate/emergent need, I.E. a mass casualty incident or abnormally high call volume, the two paramedics assigned to the fly cars potentially could place an additional ambulance into service. Albeit reducing the number of available paramedics from 2 to 1. But by this point there should be appropriate arrangements being made by EMS supervisory staff to utilize off duty volunteers to respond in to increase staffing, negating that need for the paramedics to make this change.

    If we applied this staffing model to all of our shifts and used the evidence of call volume and time periods that “mutual aid” is frequently being called, we could adjust our staffing numbers during peak time periods. There is little to no reason for another agency to take on a responsibility and cost that EMS is responsible for providing.

    Solving the mutual aid issue is something staffing on Saturdays has proven is easily rectified. EMS also needs to make a reasonable effort on recruitment and retention of its volunteer staff. Make an effort to use a combination of volunteer and paid staff to provide the maximum service during the time periods where we know mutual aid is being called while utilizing personnel so that EMS provides the highest level of service at all times. In my opinion we are not making reasonable enough effort in any of those areas.

    I am a 24 year resident of Stratford and volunteer member of Stratford EMS since 2002, I have taken advantage of every opportunity that Stratford EMS has afforded me since then, and risen through the levels of EMT-Basic to EMT-Intermediate and in 2009 I was licensed as a Paramedic after completing the 16 months of training. I am also one of the departments six volunteer Lieutenants and I fill a dual role as a volunteer Paramedic and work as one regularly. I have a Bachelor of Science from Southern Connecticut State University with concentrations in computer science, public health administration and psychology. I have dedicated a large portion of the past decade to accumulating over 7,500 volunteer hours to our community. I am also a current member of the EMS Funding Committee, so I’ve been able to analyze the department’s numbers on a budgetary and operational level.

    I will be happy to speak to anyone who would like a greater explanation of how EMS operates, and why it operates as it does. Many times there are misunderstandings that lead to perception being reality. In my humble opinion as a resident and volunteer with EMS I fully believe that EMS is capable of make the changes that are necessary to meet the needs of Stratford. The Volunteer system can and does work. Residents need to know that we have some of the best and most skilled EMT’s and Paramedic’s Connecticut has to offer.

    While there are areas within EMS that are in dire need of being fixed, first and foremost: staffing levels. Stratford EMS can continue to do this with ZERO cost to the tax-payers. EMS is self-funded and budget neutral and most commonly overlooked, we are neighbors helping neighbors.

    Another debt of gratitude and kudos goes to you for being courageous enough to identify the problem without bashing anyone individual and offering a well thought out and very viable solution. It is a fair and well thought through plan. I hope you have applied for the opening, your honesty, fairness, thoughtful actions and planning are something that the Town can certainly use. Unfortunately Stratford still continues to suffer from a case of Mironitis-it is like a horrible case of swine flu that we just can’t seem to get rid of(not bashing, just stating a factually based opinion).

    comment about EMS, everything in stratford, and all across
    the USA, is that political decisions appear to be or have been
    “DISASTER BY DESIGN.”

    Think about that?

    I wondered how the US was over run with DRUGS and so many other
    stuff not nearly as prevalent in 1950s?
    Watching Stratford and US politics and personnel, nothing surprize me
    ANYMORE … and they are becoming outright brazen…

    Timetable situation:

    Please claim the new Governor is 1st cousin to Council Chairman.

    Different people pointed out Governor’s son was in Drug trouble
    last year. Some claim son is leverage / sword of Damacles.

    In the eleven months of his term Harkins has…wait, gimme a minute.

    We are now in the 17th month of McNeil and Soto’s suspension without pay while the State Attorney continually delays the trial thereby increasing the punishment. Chief Buturla is still out of control doling out enormous amounts of overtime to his favorites just before their retirement.

    There are Assistant Fire Chiefs who are now working 36 hour shifts because there is no process in place to hire an additional assistant fire chief. In the meantime the Fire Chief and Deputy Fire Chief continue to put in almost 40 hours/week (no overtime for them in their contract).

    EMS volunteers just don’t want to volunteer anymore and apparently (if you listen to the bragging) an underqualified political appointment as EMS Chief is in the works.

    Anyone feeling the pride yet?

    Since January 7th? We don’t even have an estimate yet? Didn’t we just pay $800,000 for these two pieces of equipment? No one can get a cost of repair in 60 days?

    Solution: If you see a firefighter standing on the curb with his thumb out, give him a ride to the fire. It’ll probably be just a few minutes out of your way. Unless it is your house. Then it’s right on your way.
